Have both of your dogs testicles descended? (confirm this by seeing and feeling both testicles)(Required)
Pre-anesthetic bloodwork is recommended but not mandatory. It helps ensure your pet’s liver and kidneys are functioning properly and that blood cell counts are adequate for healing. This service is available for an additional fee of $145 and must be completed within one week of the surgery date. Alternatively, your regular veterinarian can perform the bloodwork and send us the results within 2 weeks of the appointment. Please note that bloodwork does not guarantee your pet will not experience an anesthetic, surgical, or vaccine reaction. Please indicate below if you would like to accept or decline pre-anesthetic bloodwork. If you choose to decline this testing, you will also be asked to confirm when signing the surgical admission form, acknowledging that you understand the risks associated with surgery and are confident in your pet’s health.
Has your dog received any vaccines in the last year?
We recommend your pet be up to date on vaccinations prior to surgery. Due to the clinical environment, there is a risk of exposure to infectious disease. By proceeding without current vaccinations, you accept this risk and agree to follow up with your regular veterinarian if your pet becomes ill. The Ontario SPCA cannot provide treatment for any infectious disease acquired during your pet’s visit. Vaccines are available to added on to your surgical appointment and will be administered while your pet is under anesthesia. Please note that administering vaccines under anesthesia may delay the detection of adverse reactions, and additional booster vaccines may be required for full protection.
Does your dog meet the "ideal" body condition as per the chart linked below?(Required)
